A case of phacolytic glaucoma with anterior lens capsule disruption identified by scanning electron microscopy

چکیده
BACKGROUND Phacolytic glaucoma is induced by lens protein or macrophages that have leaked through a macroscopically intact anterior lens capsule. Here, we report a case of phacolytic glaucoma with anterior lens capsule disruptions visualized by scanning electron microscopy (SEM). CASE PRESENTATION A 71-year-old man was referred to our institute for increased intraocular pressure (IOP) in the right eye. Slit-lamp biomicroscopic examination revealed corneal edema, the presence of inflammatory cells and iridescent crystalline in the anterior chamber, and a hypermature cataract in the right eye. Despite treatment with topical glaucoma medication (0.15% brimonidine, 1% brinzolamide/0.5% timolol, and 0.03% bimatoprost) and systemic mannitol, his IOP remained uncontrolled. Light microscopy was used to examine the aqueous humor obtained via anterior chamber paracentesis and the anterior lens capsule obtained via intracapsular cataract extraction (ICCE), which revealed that the anterior lens capsule was intact. However, SEM revealed full-thickness disruptions in the anterior lens. CONCLUSION This is the first reported case of phacolytic glaucoma with disruptions of the anterior lens capsule confirmed by SEM.

DESCRIPTION A 65-year-old man presented with bilateral progressive diminution of vision since the past 2 years. He had loss of vision and pain in the right eye since 2 months. He had no history of acute red eye/trauma/surgery. He had visual acuity of perception of light with accurate projection of rays in the right eye. There was white fluffy material pouring from the lens and settling in the a...

The differential diagnoses for phacolytic glaucoma are acute angle closure glaucoma, open angle glaucoma with uveitis, neovascular glaucoma, and glaucoma secondary to trauma. We report an unusual case where the dislocated cataractous lens firmly adherent to the corneal endothelium evoked a cellular reaction similar to phacolytic glaucoma but clinically appeared like a deep corneal abscess. The ...

Aqueous humor was obtained by paracentesis at the time of cataract surgery from six patients with phacolytic glaucoma, diagnosed on the basis of acute unilateral open-angle glaucoma associated with an apparently leaking hypermature or mature cataract, and from six control patients with immature cataracts. Three of the latter had primary open-angle glaucoma. Quantities of heavy-molecular-weight ...

The anterior lens capsule in pseudoexfoliative (PE) disease has been investigated with scanning electron microscopy and transmission electron microscopy. It shows several distinct zones which correlate with those seen clinically. True exfoliation of the anterior lens capsule is shown as an integral pathologic feature of PE disease. PE fibrils are mixed with various degraded cell products, inclu...
